Voluntary Dental Insurance - 2006-2007

The School's insurance provider is offering dental insurance to CST students. This insurance coverage is voluntary - you are not required to have it, but you may find it valuable.

SmileSaver Group Dental Plan 3000

To enroll yourself (and your dependents), follow these simple steps:

1. Access the website www.renstudent.com/smilesaver
2. Follow the online enrollment instructions.
3. Be sure to select a dentist and orthodontist and vision doctor from the Directory of Participating Dental and Vision Offices (go to www2.smilesaver.com/provloc.asp) and enter their code number where indicated.

Those whose enrollment information and membership fee are received on or before the 20th of the month will have an effective date of the first day of the following month. Those who join after the 20th of the month will have an effective date of the first day of the second month thereafter. (Example: if your enrollment/payment is received on 9/20/06, your coverage will be effective on 10/1/06, but if your enrollment/payment is received on 9/21/06, your coverage will be effective on 11/1/06.)

Coverage for this plan does not extend beyond September 30, 2007. Please do not submit payment for enrollment in the plan beyond this date. Should you wish to enroll for a longer time, the program for the 2007-2008 year will be available prior to the beginning of the fall quarter.

MONTHLY MEMBERSHIP FEE

You must purchase membership for a minimum of three (3) months.
Member: $12 per month
Member + One: $20 per month
Family: $27 per month
